Vertebroplasty, research design, and critical analysis.

چکیده
RECENT statements from the Association of American Medical Colleges and the Howard Hughes Medical Institute urge medical schools to design a major overhaul of educational curricula with the goal of achieving greater scientific competency of physicians-in-training (1). Interventional radiologists, like all physicians, must be able to critically analyze the medical literature and draw conclusions relevant to their practice. In particular, readers should be able to critically appraise published trials for their research design, results, validity, and generalizability (2–4). Two recent trials on vertebroplasty published by the New England Journal of Medicine (5,6) illustrate these issues quite well. The studies by Buchbinder et al (5) and Kallmes et al (6) represent a useful addition to the growing body of literature analyzing vertebroplasty. However, as is often the case, these first published randomized trials of a relatively new treatment are the starting point for the discussion, and are by no means the final word on the proce-

Percutaneous Vertebroplasty in Iranian Patients with Osteoporotic Vertebral Fractures
Background: Osteoporotic compression vertebral fractures are common clinical problems. In those with refractory fractures, percutaneous cement augmentation has been suggested. The aim of this study was to evaluate the functional outcome of percutaneous vertebroplasty in Iranian patients with refractory osteoporotic fractures. Methods: We retrospectively studied 37 osteoporotic fractures in 28 p...
متن کاملEffectiveness of vertebroplasty using individual patient data from two randomised placebo controlled trials: meta-analysis
OBJECTIVE To determine whether vertebroplasty is more effective than placebo for patients with pain of recent onset (≤ 6 weeks) or severe pain (score ≥ 8 on 0-10 numerical rating scale). DESIGN Meta-analysis of combined individual patient level data. SETTING Two multicentred randomised controlled trials of vertebroplasty; one based in Australia, the other in the United States. PARTICIPANT...
متن کاملترمیم مهره از طریق پوست( Percutaneous Vertebroplasty)
Percutaneous vertebroplasty is a promising therapeutic technique for pain control in patients with pathologic fractures of vertebral bodies. Percutaneous vertebroplasty is an appropriate treatment for vertebral pathologic fractures, which is resistant to other usual treatment. It is a useful and only choice in special cases, because of less tissue damage and there is no risk of open surgery and...
متن کاملKyphoplasty versus vertebroplasty for painful osteoporotic vertebral compression fractures—which one is better? A systematic review and meta-analysis
BACKGROUND Whether kyphoplasty or vertebroplasty is better for painful osteoporotic vertebral compression fracture is a widely debated issue. Studies on the comparison of the 2 approaches are relative limited and a wide variation exists in the patient population, study design, and results. These factors make it difficult for workers in this field to know the exact value of the 2 approaches. O...
متن کاملEffect of Augmentation Material Stiffness on Adjacent Vertebrae after Osteoporotic Vertebroplasty Using Finite Element Analysis with Different Loading Methods.
BACKGROUND Vertebroplasty is an effective treatment for osteoporotic vertebral fractures, which are one of the most common fractures associated with osteoporosis. However, clinical observation has shown that the risk of adjacent vertebral body fractures may increase after vertebroplasty. The mechanism underlying adjacent vertebral body fracture after vertebroplasty is not clear; excessive stiff...
